According to Surviving Tanks' PDF on Croatian improvised armor, it's a Tatra 138 6x6 chassis, and the damage is from a 130mm shell. It looks like a lot of these vehicles are still around as memorials.

That makes a lot more sense to me, as it looked more like frag damage then bullet strikes to me.
